Health Care Summaries and The NHS App

All care Leavers will be offered a health care summary (HCS) as they turn 18 by the CLA Team . This is a recommendation of the NICE Guidelines. This is our offer to you.

The CLA Nurses and Paediatricians will be asking young people at their Initial Health Assessment if they are 17, or final review Health Assessment if they would like a health care summary as they turn 18. If so the HCS will be completed as they turn 18.

The young people that are 17 and 9 months when they have their Review Health Assessment will have the HCS included as part of the assessment.

Leaving Care Workers (LCW) will also be promoting the NHS App with young people to help track their health histories. This app is downloadable via your smart phone and can be demonstrated by your LCW.
